The file linked below is a copy of the online registration form for the 2009/10 Year Long Program. You can use the document for reference as you gather the information needed to complete the online registration.

Note: You will be requested to copy your budget and proposal as "text " in the online application, Please forward your formatted budget and proposal documents as email attachments to roadmap@u.washington.edu

You will be reminded to print the catalyst registration form before "submitting" and then obtain signatures as requested.

Mail printed online registration form with original signatures to: Patrick J. Gibbs, Program Operations Specialist, Institute for Translational Health Sciences, Administrator, Box 358051, University of Washington, Seattle WA 98195-8051, or drop off in
T-L1 application box in the Center for Health Sciences Interprofessional Education office, Health Science Building, Room E-305.
